Baseline Tuberculosis Screening and Testing for Health Care Personnel TB Prevention in Health Care Settings

Please see FDA guidance on the use of at-home COVID-19 antigen tests. Negative results do not rule out SARS-CoV-2 infection and should not be used as the sole basis for treatment or patient management decisions, including infection control decisions. For example, specific antibody tests can be used for public health surveillance. Antibody testing is not currently recommended to assess a person’s protection against SARS-CoV-2 infection or severe COVID-19 following COVID-19 vaccination or prior infection, or to assess the need for vaccination in an unvaccinated person. A negative antigen test in individuals with signs or symptoms of COVID-19 should be repeated following FDA recommendations or confirmed by NAAT.

If you received the TB vaccine

Additionally, if your weight changes or facial/dental alterations occur, a fit test should be done again to ensure your respirator remains effective. Because each brand, model, and size of respirators will fit slightly different, you should also be fit tested every time you wear a new model, manufacturer type/brand, or size. OSHA requires an annual fit test to confirm the fit of any respirator that forms a tight seal to your face before you use it in the workplace. During a quantitative fit test, you perform a series of simple exercises that help ensure that the respirator seals well to your face even when you are speaking or moving. NIOSH and OSHA created a document with information on preparing solutions for qualitative fit testing. Instances may arise in which there is a shortage in supply of fit testing solutions.

The skin test reaction should be read between 48 and 72 hours after administration by a health care worker trained to read TB skin results. If the TB skin test is used for baseline testing of U.S. health care personnel, use two-step testing. However, the TB skin test may stimulate the immune system, causing a boosted reaction with a positive result after subsequent tests. When given a TB skin test years after infection, these persons may have little or no reaction, giving a negative skin test result. TB blood tests are the preferred test for people who have received the BCG vaccine, including children. BCG vaccination does not induce positive results when TB blood tests are used.

What to do with test results

Share sensitive information only on official, secure websites. The Occupational Safety and Health Act of 1970 established NIOSH as a research agency focused on the study of worker safety and health, and empowering employers and workers to create safe and healthy workplaces. There are companies and organizations that provide training on performing a fit test. Fit testing ctextoken.io/ is important to ensure the expected level of protection is provided by minimizing the total amount of contaminants that leak into the facepiece through the face seal. All persons with clinically active or presumed TB disease should be reported to the local or state health department.

  • Positive viral test results indicate current infection and the person with COVID-19 should take steps to prevent spreading COVID-19 to others.
  • Quantitative fit testing requires a hole punched in the respirator to perform the test.
  • Additionally, if your weight changes or facial/dental alterations occur, a fit test should be done again to ensure your respirator remains effective.

An induration of 15 or more millimeters is considered positive in:

Ask your health care provider to give you a written record of your positive TB blood test result. Health care settings in the United States should have a TB infection control plan., including TB screening and testing of health care personnel. Using a TB blood test for baseline testing of health care personnel does not require two-step testing. Ask your health care provider for a written record of your positive TB skin test result. If you have ever received a vaccine for TB (BCG vaccine), your health care provider should recommend the TB blood test.

Most people with COVID-19 have mild illness and can recover at home. Additionally, consider other illnesses with similar symptoms that may require testing. See FDA’s list of In Vitro Diagnostics Emergency Use Authorizations for more information about the performance and interpretation of specific authorized tests.

However, the second positive test reaction is actually a boosted reaction due to TB infection that occurred a long time ago. Unlike the TB skin test, TB blood tests are not affected by the TB vaccine. If you receive a TB skin test, you will have two visits with your health care provider. Your health care provider will discuss your diagnosis with you and recommend treatment. If you have a positive TB blood test result, you have TB germs in your body.

Because loose-fitting air-purifying respirators (i.e., respirators with loose-fitting hoods or helmets) do not require fit testing, they may be worn with any style of facial hair and do not require fit testing. Quantitative fit testing requires a hole punched in the respirator to perform the test. A quantitative fit test uses a fit testing instrument(s) to provide quantitative, or numerical, measurements of the amount of face seal leakage present when you wear a respirator. Fit testing is needed to determine if a particular size and model of respirator provides you with an acceptable fit.

Tell your health care provider if you have received the TB vaccine, especially if you are getting tested for TB infection. If a patient has symptoms of TB disease, health care providers should not wait for TB skin test results before starting other diagnostic tests. TB skin tests should not be performed on people who have written documentation of a previous positive TB test result (TB blood test or TB skin test) or treatment for TB disease. Treatment is strongly encouraged for health care personnel diagnosed with latent TB infection. Some people with latent TB infection have a negative reaction to the TB skin test when tested years after being infected. Two-step testing is recommended for the initial TB skin test for adults who may be tested periodically, such as health care personnel.
